RN- Registered Nurses and LVN- Licensed Vocational Nurses provide a wide range of services including, wound care, IV infusions, medication reconciliation and management, catheter care, ostomy care, monitoring vital signs, education on new treatments, Diabetic care, Respiratory care, Parenteral feedings, monitoring and carrying out any treatment ordered by a physician or allowed practitioner.
Physical, Occupational or Speech Therapists- PT, OT and ST’s help patients regain mobility, strength and independence due to decline in functional ability to perform regular activities of daily living including safe ambulation, bathing, dressing, grooming, eating and toileting. Poor endurance and uncontrolled pain causing loss of function and decline often follow a fall or hospitalizations due to acute episode or chronic illness. The goal is to restore optimal ability to function and keep patient as independent as possible. A treatment plan is developed following an evaluation ordered by a physician or allowed practitioner.
Aides help patients with personal care including bathing, grooming and toileting on intermittent bases when there is no caregiver able to assist while the patient is receiving home health skilled services.
Social workers help patients with arranging community resources, counseling as needed and emotional support to patients and families in the comfort of their home as they face acute or chronic illness.
