HOME HEALTH CARE

Home health skilled services are provided in the comfort of your home with the goal of helping patients regain independence, strength, and mobility while nursing the individual back to health and/or prior level of functioning.


Home health benefits people of all ages and acute or chronic medical conditions who are home-bound either temporary or permanent in need of intermittent skilled nursing care, skilled Physical, Occupational or Speech therapy, home health aides to assist with activities of daily living and Clinical social workers to assist with community resources and emotional support if needed. The goal is to restore and preserve functional ability and independence as much as possible.


What is Home-bound?

Patients may qualify for home-bound status when challenged with a life-limiting illness or temporary condition whether chronic or acute in nature, subsequently causing a setback in their ability to function independently. Often resulting in needing assistance with ambulation, making it unsafe to leave home unattended, causing a taxing effort to leave their home for skilled treatments, taking medications that alter consciousness by increasing the risk of falls and safety, having no one at home able to assist at the time, needing personal care and supervision, having cognitive  impairment, low vision, impaired motor skills, functional decline and other conditions caused by underlying chronic or acute illnesses.

What is included in home health services?

Skilled Nursing care

RN- Registered Nurses and LVN- Licensed Vocational Nurses provide a wide range of services including, wound care, IV infusions, medication reconciliation and management, catheter care, ostomy care, monitoring vital signs, education on new treatments, Diabetic care, Respiratory care, Parenteral feedings, monitoring and carrying out any treatment ordered by a physician or allowed practitioner.

Skilled Therapy Services

Physical, Occupational or Speech Therapists- PT, OT and ST’s help patients regain mobility, strength and independence due to decline in functional ability to perform regular activities of daily living including safe ambulation, bathing, dressing, grooming, eating and toileting. Poor endurance and uncontrolled pain causing loss of function and decline often follow a fall or hospitalizations due to acute episode or chronic illness. The goal is to restore optimal ability to function and keep patient as independent as possible. A treatment plan is developed following an evaluation ordered by a physician or allowed practitioner.

Home Health Aides

Aides help patients with personal care including bathing, grooming and toileting on intermittent bases when there is no caregiver able to assist while the patient is receiving home health skilled services.

Licensed Clinical Social Worker

Social workers help patients with arranging community resources, counseling as needed and emotional support to patients and families in the comfort of their home as they face acute or chronic illness.

Chronic Care Management

Home Healthcare professionals assist in managing chronic conditions such as Diabetes, heart disease, respiratory disorders and post-acute Transitional care ensuring proper communication between all disciplines involved, ensuring proper patient/family education and medication/treatment adherence. Monitoring treatment progression and changes as ordered by a physician or allowed practitioner.

Hospice care

What is Hospice?

Hospice care is a program that treats the person with mercy and kindness when the patient is facing a life-limiting disease. Hospice care focuses on the quality of life for individuals who are experiencing an advanced life-limiting illness. It is intended to support the patient in the last 6 months of life. Hospice care addresses the patient's physical, emotional, social, and spiritual needs.


What is Hospice philosophy?

  • Hospice care treats the person and symptoms of the disease, rather than treating the disease.
  • Hospice care DOES NOT hasten or postpone death.
  • Hospice accepts death as a final stage of life.
  • The Hospice team takes care of every patient.
  • The team consists of Medical Directors, skilled nurses, home health aides, Advanced Nurse Practitioners, Chaplains, Licensed Social Workers, volunteers, and bereavement support.


How are these services covered and in what setting?

  • Most health insurance, including Medicare, covers hospice services in any setting. Medical equipment and medications related to the end-stage diagnosis of the patient for which hospice care is provided are also covered.
  • Hospice care services can be provided in the comfort of the patient's home, skilled nursing facilities, Assisted Living facilities, or hospitals.


Who cares for the Hospice patient? 

  • Hospice care also provides Respite Care for 5 days per benefit period with the goal of providing relief to the patient's caregivers.
  • Continuous Care when the patient qualifies for a higher level of care due to a health decline in accordance with CMS regulations.
  • Delivery of care in an Inpatient Unit may be appropriate provided the patient qualifies for a higher level of care requiring skilled nursing care around the clock.

  

When is Hospice Care appropriate?

When a life limiting condition has adv advanced resulting in functional decline, and the patient has increased failure to thrive. Some examples include but are not limited to weight loss, decreased functional mobility, declined cognitive ability.

Palliative Care services is also provided in the comfort of your home or place of residence, the goal is to enhance comfort and provide symptom management by improving the quality of an individual’s life who is facing a chronic/serious illness but may not qualify or meet criteria for Hospice services. It’s appropriate for any age and/or stage of a chronic/serious illness along with the treatment that is meant to cure the illness.
